Operation Steps

Add Contact Question

Feature Entry
「Edit」 tab → 「Other types of questions」 → 「Contact」 button

1. Click the top 「Edit」 tab
2. Expand the 「Other types of questions」 area in the left panel
3. Click the 「Contact」 button to add it to the form

Allow Selecting Multiple Contacts

Contact question → 「Contact Settings」 → 「Allow selecting multiple contacts」 toggle

1. Click the added contact question
2. In the right 「Contact Settings」 panel, turn on the 「Allow selecting multiple contacts」 toggle

Set Default Value

Contact question → 「Contact Settings」 → 「Default Value」 option

2. In the right 「Contact Settings」 panel, click the 「Default Value」 option
3. Select: None / The person filling out this form / Specified person
